Loops ,waves, tables

Loops, flow and pressure waves give invaluable information about lung compliance and resistance to flow and should be routinely assessed for all patients as a means of individually tailoring ventilation therapy. There are numerous waveforms and loops that can be assessed. We prefer the Paw – V loop and the V – flow loop and we determine the pressure, volume, flow or CO 2 waveform according to individual patient requirement. The pressure and volume waveforms are particularly useful when using the ACT function. Learn to observe your waves and loops in response to parameter changes, bronchodilator therapy and clinical changes in patient’s condition.

Waves

Paw, Flow, V, CO2 are accessible by pressing the waveform icon. When ACT enabled a green line appears in Paw wave, which indicates the pressure at the carina. This is a valuable tool to use for ET tube malposition.

Loops

The loops are accessed through selecting the Values measured button and then selecting the loops icon on the touch screen. On loop icons have six available loops each Paw-V, Flow-Paw, V-Flow, V-CO2, Ptrach-V, Flow-Ptrach.

TABLES 1 & 2

By selecting the values measured button and the table icon on the touch you have access to breath by breath parameters.

Indications for weaning

· resolution of process requiring ventilation

· clinically and radiologically improving lung disease

· appropriate conscious state

· haemodynamic stability

· stable metabolic and acid base parameters

· rapid wean in most cases

· objective parameters oxygen/ventilation indices

Weaning modes

· BIPAP + /_ Apnoea ventilation

· CPAP + /_ Apnoea ventilation

· SIMV (MMV) + PPS with reducing rate and cm PPS

· T-piece weaning

Wean to Premorbid ABG FiO 2 0.5, PaO 2 60-80 mmHg, PaCO 2 < 50 mmHg, RR <>
