Star Infrasonic ventilator
Classification:

Modes available:
· volume control (SIMV + PPS, ASSIST CONTROL, CPAP & PPS )
· pressure control Time cycled (SIMV + PPS, ASSIST CONTROL, CPAP & PPS)
· pressure control I:E cycled (SIMV + PPS, ASSIST CONTROL, CPAP & PPS)
· BIPAP
Quick test: Mandatory prior to initiating ventilation
· depress and hold down visual reset and alarm silence buttons while turning the panel ON/OFF switch to ON
· after 5 secs the performance, safety and diagnostic screen will be displayed
· move the cursor to quick test and press enter
· follow prompts (there are fourteen tests that are automatically performed during the quick test; if a test fails, use the cursor to rerun this test, once you’ve rectified the problem)
Default ventilator setting Volume control SIMV 10 BPM x TV (10 mls per kg) x
FIO 2 .5 x Peep 5cms x PPS 20 cms Flow LPM 60 x decelerating waveform.
Select mode Volume control
SIMV + PS
Settings:
· set tidal volume at 800 mls
· set the intermittent mandatory ventilator rate at 8
· set flow rate at 60 LPM
· set trigger sensitivity at 2 cm
· set pause 0.5 sec
· set pressure support at 20 cm H 2 O
· peep 5 cm H 2 O
· FiO 2 .5 until adequacy of ventilation checked by blood gas analysis
· decelerating waveform
· sigh rate 0
· apnoea rate 8
Alarm limits:
· high inspiratory pressure limit 35 cm H 2 O
· low inspiratory pressure limit 10 cm H 2 O
· low mechanical volume 400 mls
· low spontaneous volume 0
· low minute volume 4L
· low PEEP/CPAP 2cm
· high respiratory rate 26 bpm
· apnoea 20 secs
Measurement of Auto Peep
· autopeep must be measured if inverse I:E ratio (in controlled modes only)
· select screen change and enter
· toggle to respiratory mechanics and enter
· toggle to Autopeep and enter Autopeep will be measured over three breaths
Measurement of NIF (must be > 20 cm H 2 O)
· NIF must be measured prior to extubation (in spontaneous modes only)
· select screen change and enter
· toggle to respiratory mechanics and enter
· toggle to NIF and enter
· enter while instructing patient to breath in
· only repeat if <> cm H2 O as very uncomfortable for the patient
Measurement of SVC (must be > 10 mls/kg)
· SVC must be measured prior to extubation (in spontaneous modes only)
· select screen change and enter
· toggle to respiratory mechanics and enter
· toggle to SVC and enter
· enter while instructing patient to breath out
PRESSURE CONTROL I:E CYCLED
· select pressure control I:E cycled and enter
· select assist control and enter
· select I:E 1:2 and enter
· select rate 8-12 bpm and enter
· select flow 160 LPM and enter
· select Oxygen 50 % and enter
· select PEEP/CPAP 5cm and enter
· select Sensitivity at 2 cm and enter
· select apnoea rate 8 bpm and enter.
· toggle down to right hand corner of the screen and select start
The Pressure control mode will now be enabled.
PRESSURE CONTROL TIME-CYCLED
· select pressure control Time-cycled and enter
· toggle to select (assist control or SIMV + PPS or CPAP + PPS) and enter
· select PCV pressure at 10-20 cms H 2 O or equivalent to plateau pressure in SIMV mode
· select insp. time at 1.5 and enter
· toggle to select rate 8-12 bpm and enter
· toggle to select flow 160 LPM and enter
· toggle to select oxygen 50 % and enter
· toggle to select PEEP/CPAP 5cm and enter
· toggle to select Sensitivity at 2 cm and enter
· toggle to select PPS 0-20 cms H 2 O
· select apnoea rate 8 bpm and enter
· toggle down to right hand corner of the screen and select start
The Pressure control mode will now be enabled.
CPAP +PPS ( BIPAP )
Weaning/assist mode.
· select volume control
· select CPAP + PPS
· set tidal volume at 10 mls/kg
· set the intermittent mandatory ventilator rate at 8-12
· set flow rate at 60 LPM
· set pause 0.5 sec
· set pressure support at 20 cm H 2 O
· Peep 5 cm H 2 O
· FiO 2 .5 until adequacy of ventilation checked by blood gas analysis
· decelerating waveform
· sigh rate 0
· apnoea rate 6
