Treatment for tension pneumothorax

A 16G cannula is placed in the mid clavicular line, second intercostal space to relieve a tension pneumothorax. An UWSD will always be inserted following this procedure to reinflate the collapsed lung.

Heimlich valve

May be used as an alternative to an UWSD bottle. This is a one way valve, which allows air and fluid to escape from the chest on expiration, but prevents air being entrained back into the chest during inspiration.

· may be used in an emergency situation (located in the Emergency Trolley)

· it is useful for ambulating patients with an air leak only

Disadvantage: may be prone to blockage by clots.

NB Ensure the heimlick valve is attached in the correct direction, as this is a LIFE THREATENING situation if not. Never connect a wound drainage bag to a heimlich valve
